LOCATION: Ontario, Yates, Seneca, and Livingston Counties
DEPARTMENT: Home Care
SCHEDULE: Monday through Fridays, Days
ATTRIBUTES
Masters degree in Speech/Language Pathology (may be MA, MS, Med)1 year experience as a SLP preferred. Home Care Only: Current N.Y.S. Patient Evaluation & Development. Meet the patient's goals and needs by providing quality, evidence based care; assess, develop and implement an individualized treatment planPatient Care. Help patient accomplish treatment plan and accept therapeutic devices/recommendations; instruct, encourage and assist patients in performing daily-living activities and in using assistive and supportive devices Leadership. Develop the speech-language therapy staff; provide information, develop and conduct in-service training programsEDUCATION:
MS: Speech Pathology (Required)LICENSES / CERTIFICATIONS:
CCC-SLP - Certificate of Clinical Competence in Speech-Language Pathology - American Speech-Language-Hearing Association (ASLHA), SLP - Speech-Language Pathologist - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.
