Role and Responsibilities Direct, supervise, evaluate and provide speech therapy services to patients in the home, as prescribed by the attending physician. Essential Functions + Evaluate the patients speech, language and voice, and swallowing abilities according to referral and perform periodic reevaluation. + Develop and implement rehabilitative services for speech, language and voice, and swallowing disorders. + Select and administer diagnostic tests and provide appropriate therapeutic techniques and materials. + Record the type of treatment and patients reaction to it on the clinical/progress notes. + Maintain adequate records on all of the patients, including summary reports. + Instruct and educate family members and patient, regarding treatment plan and therapy carry-over. + Attend case conferences. + Participate in the development of and periodic revision of the physicians plan of care. + Other duties as assigned.
