The SLP specializes in assessment and diagnosis; intervention, management, counseling, and follow-up services for disorders of speech, language, cognitive aspects of communication, swallowing or other upper aero digestive functions. Provides this service in a home setting while performing the following duties:

Reviews Physician's referral, patient's condition and medical records to determine speech therapy treatment required. Obtains history and subjective components of the evaluationIn the home setting: Diagnoses and evaluates speech, language, communicative, cognitive, and swallowing skills as related to educational, medical, social, and psychological factorsPlans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication problems of organic and nonorganic etiologyProvides counseling and guidance and language development therapy to patientsReviews individual file to obtain background information prior to evaluation to determine appropriate tests and to ensure that adequate information is availableAdministers, scores, and interprets specialized hearing, speech and language testsDevelops and implements individualized plans for assigned clients to meet individual needs, interests, and abilitiesReviews treatment plan, and assesses individual performance to modify, change, or write new programsEvaluates swallowing abilities via bedside swallow/video studies, developing a plan of care based on findings and individualized patient needsMaintains records as required by law, establishment's policy, and administrative regulationsPerforms complete, thorough, and timely electronic documentationInstructs individuals to monitor their own speech and provides ways to practice new skillsActs as consultant to educational, medical, and other professional groupsConfers with Physician and other practitioners to obtain additional patient information, suggest revisions in treatment program, and integrate speech therapy treatment with other aspects of patient's health careParticipates in the PI and CQI process through committees as neededAssist in the orientation and training of new staff

Requirements:

Licensed Speech Therapy in the State of IllinoisApproved CPR or BLS certification required within 90 days of hireMaintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.Have and maintain an acceptable driving record per Mercy's HR-01 Fleet Safety policy


Education and Experience

A qualified speech-language pathologist holds a Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing AssociationMaster's Degree from an accredited Graduate Program.Completion of a Clinical Fellowship under the supervision of a certified SLPHome health experience preferred
