Essential Duties and Responsibilities: 

Provides direct and consultative speech therapy services, including assessments and treatments in accordance with regulations, standards, and guidelines defined by the American Speech/Language and Hearing Association (ASHA), each patient/resident’s plan of care, and established departmental and facility policies and procedures

 

Screens new admissions, re-admissions, and requests by nursing to deem if appropriate for evaluation. 

 

Established, develops and implements a therapy plan for each client, maintains client progress notes, commenting at least once a week and modifies therapy plans and objectives in accordance with client progress. 

 

Counsels family of clients receiving Speech Language services. 

Utilizes a system of documentation that is professional, efficient, accountable, and conforms to federal, state, and facility policies and procedures; develops and maintains progress reports, session notes, treatment summary reports, etc. 

 

Facilitates a team approach to program development, implementation, and monitoring by effectively collaborating with all specialty/clinical disciplines, family members, etc., to identify and best meet the speech therapy needs of each patient/resident. 

 

Prepares a discharge report including background information, summary of therapy, and disposition at time of clients discharge from Speech Language Pathology. 

 

Follows and enforces strict adherence to safety and infection control rules, regulations, policies and procedures. 

 

Performs all tasks and duties in an efficient and safe way. 

 

Performs other related duties as assigned or as necessary. Remains flexible and adaptable in work schedules and work assignments as defined by departmental and facility needs.

EDUCATION AND/OR EXPERIENCE 

 

Master’s Degree in Speech Language Pathology. 

 

A Clinical Fellowship Candidate or Speech Pathologist with at least 1-year experience with a geriatric population. 

 

Must have a sincere desire to work with the aged, disabled and infirmed and a willingness to function as part of an interdisciplinary team. 

 

Must be able to effectively communicate with older adults in a manner that conveys warmth and professionalism 

 

Licenses and/or Certification: 

Certificate of Clinical Competence awarded by the American Speech Language-Heading Association or has applied for it. 

 

 

Must be licensed as a Speech/Language Pathologist by the State Board of Examinations in Speech-Language and Hearing or excluded from the requirements of licensure under the Speech-

Language and Hearing Licensure Act.
