Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.
The Spiritual & Psychosocial Coordinator is responsible for the coordination and supervision of all social work, chaplain, volunteer and bereavement disciplines to ensure services are consistent with the mission, vision, and values of McLeod Hospice. Develops and implements methods for measuring effectiveness and positive outcomes for our spiritual and psychosocial services.
1. Participates in planning, organizing and directing the social work, spiritual, bereavement, and volunteer activities in consultation with Hospice Clinical Managers and Administration.
2. Coordinates staff coverage for inpatient and outpatient hospice programs in coordination with Clinical Managers.
3. Supervises BSW social workers according to Conditions of Participation.
4. Provides direct social work services to terminally ill patients and families.
5. Conducts timely evaluations and assists in goal setting and ongoing educational plans for staff.
6. Demonstrates knowledge of the Conditions of Participation, NASW Code of Ethics, licensing and other requirements for Hospice social workers, chaplains, bereavement, and volunteers.
7. Performs other duties as assigned.
Qualifications /Training:
• 3 years of work experience in hospice or related field.
• Must possess in-depth knowledge of community resources available to client base.
• Must be knowledgeable of principles of leadership and interprofessional team dynamics.
• Must have a high degree of maturity, self-motivation, initiative, and independent judgement.
Licenses/Certifications/Registrations/Education:
• MSW from a graduate school of social work accredited by the Council of Social Work Education.
• Valid South Carolina license to practice social work.
• Valid South Carolina driver’s license.
