Sr Director, Home Care - Administration - Full Time
Summary: The System Senior Director of Home Care will lead and execute the strategy for home care across the Guthrie Clinic. This position will provide administrative, operational, and business development leadership in support of assigned Homecare and At-Home services. The work will span across the boundaries of The Guthrie Clinic to ensure the effective integration of strategy and operations achieving the defined key performance indicators. This position will collaborate with physicians, case management, hospital leadership, community providers, and other service providers to enhance service delivery to patients in the continuum of care. This role reports to the Chief Clinical Officer of The Guthrie Clinic. Education: + Bachelor’s Degree required, Masters preferred. + Registered Nurse preferred. + Advanced training and industry certifications are highly preferred. Experience: + Minimum of 5 years of experience in healthcare, with at least 2 years in a leadership role focused on home care + Experience in Hospital at Home, remote patient monitoring and/or Mobile Integrated Health is highly desirable + Proven track record of successfully leading growth in home care, preferably within a large, integrated health system + Strong knowledge of home care trends, federal and state regulations, reimbursement requirements, and the evolving health care delivery system + Ideal candidate will have direct home care experience in both PA and NY + Excellent leadership, communication, and interpersonal skills, with the ability to influence and drive change across a complex organization + Strong business acumen with the ability to drive change in a large, complex organization + Demonstrated success in leading large-scale initiatives that have significant impact across the organization Essential Functions: + Develop and implement a comprehensive Homecare strategy that aligns with The Guthrie Clinic’s mission and goals, focusing on care in the home, care transitions, and readmission reduction + Lead the strategic planning, development, and deployment of homecare initiatives, ensuring they meet the needs of patients, providers, and the health system + Provides daily operational and strategic oversight in collaboration with The Guthrie Clinic key leaders and ensures alignment with broader organizational priorities. + Through a matrix reporting relationship, provides guidance and leadership, including appropriate participation in annual performance reviews + Develops appropriate plans to assure successful financial performance of the Homecare and At-Home services including analysis of various financial reports and statements + Identification of opportunities and implementation of successful interventions to increase the capture of value-based revenue through the CMS Home Health Value Based Purchasing Model and other payer arrangements + Primary liaison with Post-acute Board of Directors + Lead and/or support key committees + Accountable for the transition of care, innovation and success of operations of assigned Homecare and At-Home services. + Attention is to be given to patient satisfaction, systems improvement, program development, quality and safety, fiscal management, compliance, work culture enhancement, and internal communication and consensus building. + Ensure compliance with relevant licensing, regulatory and accreditation requirements + Develops, implements and evaluates programs and strategies designed to create and maintain safe, functional, secure, clean and orderly programs to ensure efficient, safe and economical operations. + Promotes individual professional growth and development by meeting requirements for mandatory continuing education, skills competency, supports department-based goals which contribute to the success for the organization, severs as preceptor, mentor, and resource to the less experienced staff.
