We are currently looking to hire a Registered Nurse to join our Hospice team in Essex County on a per diem basis. The person in this role will be responsible for providing skilled nursing care for Pediatric Hospice patients and families.
Responsibilities
1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
2. Performs physical assessment and evaluates the needs of Hospice patients and develops an appropriate plan of care
3. Documents all changes in client’s condition, updates care plans and outcomes
4. Identifies learning needs and plans appropriate teaching strategies for individuals and families
5. Monitors quality and appropriateness of services provided
6. Provides and follows individualized care plan set up by the case manager.
7. In communication with the RN case manager and the Hospice Medical Director on any changes or concerns one has after an assessment of the pediatric hospice patient,
8. Adheres to agency infection control and safety policies
9. Adheres to the organization’s policy in regards to absenteeism and appearance
1. Active New Jersey Registered Nurse License
2. 1+ year of pediatric Health experience
3. Excellent verbal and written communication skills
4. Strong computer skills
5. Car and valid driver’s license
Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and customer contact.
