Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy** **Responsibilities** **Now Hiring** **Full time Staff Pharmacist for the Indianapolis, Indiana location!** **Offering Great Competitive Salary and Benefit package** The Pharmacist provides safe and appropriate pharmacy services to home infusion patients. The Pharmacist accesses patients’ suitability for home infusion care, reviews and evaluates medication orders, labs and other clinically relevant information monitoring patients’ progress and making recommendations when needed. The Pharmacist is responsible for overseeing sterile compounding of medication and quality assurance. + Monitors and evaluates patient specific pharmacy care plans for the pharmacotherapy of home infusion patients. Collects and organizes all patient related data needed to prevent, detect and resolve medication-related problems. Documents outcomes in CPR+, completes company occurrence reports. Designs and modifies pharmacy care plans based on evaluated information and makes recommendations for continued monitoring when appropriate. + Receives medication orders from physician and evaluates the appropriateness of orders for dosage,drug choice, potential drug interactions, drug-food interactions, route of administration and stability. + Ensures medication specific laboratory tests are ordered and results received. Responsible for the retrieval, review and consultation with physicians and nurses of ordered labs. Utilizes critical lab, critical value report. + Prepares and dispenses medications, selecting cost effective and patient safe and appropriate methods of infusion, following Infusion’s policies and procedures. + Manages the use, maintenance and troubleshooting of all pharmacy equipment. Ability to accurately program, maintain and troubleshoot all IV medication related equipment. + Allows for the continuity of pharmaceutical care from pharmacy through the delivery process. Ability + to communicate pertinent information to the patient, caregiver and nursing department. + Provides medication-related education to Infusion staff, nurses, physicians, patients and caregivers. + Assists the physician, nurse, patient, caregiver and Infusion staff with continued technical and clinical support. Provides accurate, concise and timely responses to clinical questions from health care providers, patients and caregivers. + Accurately documents all communication with physicians, nurses, patient, caregivers and other medical personnel in the electronic medical record and patient chart. + Assists in the training and supervision of pharmacy technicians, pharmacy externs and delivery personnel. + Participates in Performance Improvement (PI) Program Coordination; including assisting with the overall development and implementation of the PI plan, assisting in the identification of goals and related patient outcomes, participation and reporting of activities and outcomes, development and implementation of corrective plans. + Participates in the on-call rotation. + Participates in the pharmacy team meetings. **Qualifications** + Registered pharmacist/State licensure in state(s) serviced. + One year hospital pharmacy experience. + Experience with and knowledge of IV admixture procedures. + Excellent communication skills: oral and written. + Flexible and able to work with a multidisciplinary team. + Excellent time management skills and able to work independently. + Ability to function well as part of team. **Pay Range** $49.52 - $71.80 /hour We are an equal opportunity/affirmative action employer.
