Responsibilities Conduct patient evaluations and create customized treatment plans Perform physical therapy interventions and provide ongoing patient care Collaborate with other healthcare professionals to ensure comprehensive patient care Educate patients and their families on treatment options and expectations Document patient progress and maintain accurate and detailed medical records Stay up-to-date with the latest research and advancements in physical therapy Assist in the development and implementation of departmental policies and procedures
Qualifications:

Graduate from an accredited Physical Therapy Program.

Candidates must possess in-state licensure

1 year of experience as a physical therapist

1 year of home health experience as a physical therapist - preferred

Candidates must possess strong caring interpersonal skills
