Implements, coordinates, and assists in developing services and activities related to the Stroke Program. Functions as a liaison between the providers, nursing staff, allied healthcare professionals, patients and their families. Coordinates and facilitates the care for the stroke patient throughout the continuum. Works closely with providers and nursing staff to ensure clinical care path is followed. Participates in program development including staff education, community education, and marketing. Responsible, in coordination with department leaders, for planning, developing and implementing stroke center initiatives. Maintains quality standards for responsive service including data management and performance improvement for key quality indicators. Assumes a key role in development and implementation of outcomes evaluation as guided by joint commission accreditation standards.
Minimum Qualifications Work Experience Three years of nursing experience including care of stroke patients. Previous stroke program experience preferred.Education Graduate of an accredited nursing program required. Bachelor's degree in Nursing, Healthcare Administration, or related field required. Master’s degree preferred.
Certifications/Licensure Kentucky RN license (OR) RN license in a State recognized by the Kentucky Nurse Licensure Compact Act required effective June 1, 2007. Assists with the coordination of multidisciplinary care for stroke patient in collaboration with providers, nursing staff, and other members of the care team. Acts as a liaison between providers, nursing departments, ancillary staff on behalf of patients and their families. Provides clinical resources and consultation to staff regarding stroke patients with clinical complex needs. Completes prospective chart reviews to ensure compliance with stroke accreditation guidelines. Leads and/or assists with coordination of performance improvement program initiatives, including outcome evaluations and action planning as necessary. Ensures that stroke care is consistent with current evidence-based best practice. Communicates with leadership regarding any issues regarding service, staff, or patients. Works with nursing leadership and educators to implement formal and informal nurse education and training regarding stroke care. Assists with development and coordination of community education for healthcare professionals and other members of the community. Represents and promotes stroke services to hospital staff, volunteers, patients and visitors. Leads regular stroke team meetings and provides program performance reports at facility leadership meetings. Coordinates activities and surveillance associated with maintaining regulatory survey readiness with regards to stroke certification. Assists in the completion of the annual computerized self-assessment tool and action plan for deficiencies prior to on-site survey. Provides administrative and staff updates and education regarding any regulatory or standards changes. Collaborates with managers to resolve continued noncompliance with standards. Provides mentoring and coaching of care-providers regarding aspects of on-site survey inspections Collaborates across entities for consistency in policies and procedures Reviews policies and procedures assuring compliance with regulatory standards Assists in the coordination of activities associated with unannounced surveys Assists in the development of responses to required evidence of standards compliance as a result of unannounced surveys Coordinates activities associated with mock survey, reporting of findings, and subsequent requested responses Directs educational activities and organizes team learning activities to support survey readiness.
