Position Summary:
We have an exciting opportunity to join our team as a Stroke Coordinator.

In this role, the successful candidate Provides co leadership with the Medical Director to coordinate the development and implementation of the Comprehensive Stroke Care Center consistent with the strategic direction outlined by the Neurology and Rehabilitation Service Lines With the Comprehensive Stroke Care Center Steering Committee establishes priorities and implements long and short term action plans toward the goals of the program Functions collaboratively with the Director of Nursing Medical Service the Nurse Manager for Neurology other nursing leadership in rehabilitation and emergency services and the attending neurologists and neurology nurse practitioners to provide care management for the stroke unit as needed Collaborates and coordinates all activities with key personnel in departments that participate in provision of services to the Comprehensive Stroke Care Center including emergency acute hospital and acute rehabilitation in patient and out patient In conjunction with the Medical Director and Hospital Administration develops and implements strategies for marketing research staff and community education

Job Responsibilities:

Collects, assesses and documents patient healthcare data.a - Involves patient/family and interdisciplinary team as appropriate. b - Prioritizes data collection.c - Documents assessment in medical records.Identifies factors and participates in programs to promote satisfaction of staff, patient, families and other disciplines.Modifies care based on new and revised nursing patient care standards.Maintains awareness of current evidence based practice and utilizes findings from current nursing literature in patient care activities.Demonstrates awareness of NDNQI/Q &PI data and implications for improving patient outcomes.Participates in unit based quality performance programs/projects.Utilizes resources to meet individual learning needs and promote development in clinical practice.a - Demonstrates awareness of resources and seeks out experts in nursing and other disciplines to promote learning and development in evidence based practice.Shares knowledge and participates in peer review process by providing information as requested.Engages in self-assessment and participates in activities to promote own professional growth and development.a - Assesses and recognizes personal learning needs to enhance performance with guidance from Nursing Leadership.b - Learns and uses new information, skills and knowledge in an effort to improve care and services.Acquires and maintains current knowledge and competence to fulfill responsibilities.a - Completes the requirements of the Nurse Residency Program.Maintains confidentiality of patient information.Serves as a support and resources for other nursing staff and students.Demonstrates awareness of the mission, philosophy, standards, goals and objectives of the NYU Langone Medical Center, Departments of Nursing, clinical department and strategic area.a - Participates in unit/service based meetings.b - Recognizes importance of cost effective patient care.c - Reports unexpected events and outcomes to senior staff/leadership in a timely manner.d - Delivers patient care in accordance with professional nursing standards.e - Demonstrates awareness of JC, DOH, CMS, and CARF standards pertinent to unit.f - Communicates effectively with patients families, visitors and all members of the interdisciplinary team.g - Delegates activities effectively and appropriately to the ancillary staff.5 - Educates the patient/family regarding health status and established plan of care.a - Determines learning needs of the patient/family.b - Implements an educational plan.c - Collaborates with the interdisciplinary team to modify expected outcomes.d - Documents learning outcomes of patient/family and revises education plan.Evaluates the patient's progress towards expected outcomes.a - Involves the patient/family and interdisciplinary team in the evaluation process.b - Documents and evaluates the effectiveness of the intervention(s).Implements interventions in the plan of care.a - Intervenes in a manner that is consistent with established plan of care.b - Implements interventions in a safe, timely and appropriate manner.c - Considers needs and behaviors of specific patient age groups and cultural groups in all patient care activities.d - Documents interventions per hospital standard.Develops plan of care to attain expected outcomes.a - Develops a nursing plan of care based on agreed upon outcomes with patient/family.b - Collaborates with the interdisciplinary team.c - Identifies outcomes that are realistic and measurable.

Minimum Qualifications:
To qualify you must have a BSN required 5 8 years clinical experience in neuroscience and/or rehabilitation nursing Active NYS RN license Specialty certification Neuroscience or Rehabilitation Nursing.

Required Licenses: Registered Nurse License-NYS

Preferred Qualifications:
Baccalaureate degree in Nursing required Masters degree preferred 3 5 years of clinical experience in neuroscience and/or Rehabilitation Nursing Prior education/teaching experience preferred Demonstration of ability to facilitate group processes and work cohesively with an interdisciplinary team Evidence of excellent interpersonal skills effective communication skills and innovative problem solving Excellent chart review and data abstraction skills

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $84,222.07 - $140,370.14 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
