Our Boise Heart Care Clinic proudly cares for patients over the age of 55 diagnosed with conditions including coronary artery disease, hypertension, irregular heart rhythms, or congestive heart failure. Our clinic includes Electrophysiology, Echocardiogram, treadmill and nuclear stress tests, and lab services.

The Structural Heart Coordinator for the Saint Alphonsus Structural Heart program is a registered nurse independently accountable for the effective management and coordination of care of patients primarily being evaluated for structural heart procedures. The Structural Heart Coordinator will partner with the interventional cardiologists, cardiothoracic surgical team, and clinical staff of multiple departments, both inpatient and outpatient within Saint Alphonsus and external local and regional healthcare facilities to coordinate the care of cardiovascular patients who are being evaluated and selected for structural heart procedures through their pre and post-procedure care.

About This Position:

Collaborates with and facilitate patient focus to promote heart failure optimal outcomes with close collaboration with entire care team.

Performs daily rounds in the hospital and/or chart reviews to verify patient needs and additional resources or additional involvement with the multidisciplinary team while preparing and transitioning patient for the next level of care. Ensures compliance with the core heart failure measures and other evidence based criteria with provider support.

General Requirements:

2 years of experience in cardiology required

Education:

An associate or bachelor's degree from a nursing program accredited by ACEN or CCNE

License/Certification:      

Must have an RN license in the state of Idaho issued prior to hire date.

Must provide licensure or proof of application in the State of Oregon within six months of starting

Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire

Must obtain ACLS within 6 months

ESSENTIAL FUNCTIONS:

Provides care coordination to patients who have valvular heart disease. The coordinator requires advanced nursing knowledge and skills to ensure patient safety and optimal clinical outcomes.

Partners with Radiology, Non-Invasive Cardiology Lab, Invasive Cardiovascular Lab, outpatient clinic and inpatient units at St. Alphonsus Health Systems to systematically and continuously collect and access data (in collaboration with the multidisciplinary team) to provide therapeutic care of the patient and family.

Participates in research studies, maintains registry data on a daily basis and provides informational communication to the multidisciplinary team.

Maintains an in-depth database by assessing the behavioral and physiologic status of the patient utilizing interview, observation, physical examination, and other available data. Ensures accurate and timely entry of data.
