As a Substance Abuse Specialist, you will provide skill trainings, case management, and crisis intervention to participants with co-occurring disorders by utilizing the team approach of Assertive Community Treatment (ACT). You will report to the Director of Act and will work onsite
Responsibilities:
Conduct all phases of psychosocial rehabilitation and outreach (engagement, assessing strengths, goal planning, safety planning, evaluating, and summarizing progress). Provide and coordinate substance abuse assessment. Create and review relapse prevention plans such as wellness recovery action plan Assist with identifying natural supports and accessing community resources that support recovery. Provide and coordinate substance abuse services tailored to needs and stage of change Coordinate care with inpatient substance abuse treatment, as needed Utilize evidenced based practices when delivering services to participants, as applicable. Actively support participant self-reliance. Provide skills training for medication management, financial management, and for all activities of daily living. Provide case management and crisis intervention as needed. Collaborate with other staff members, community agencies/supports, and family members about mutually served participants. Complete required documentation in a timely fashion. Participate in case conferences, staff meetings, and training programs as required. Act as liaison with other appropriate community agencies. Transport clients to appointments and other services as needed Must be willing to transport clients, in agency vehicles, when applicable/appropriate. Be an active part of the ACT on-call rotation. Participate in special projects and other duties as assigned.Requirements and Education:
Requires an Associates degree and at least one year of directly related experience assisting individuals with substance use and mental illness. Must have a valid drivers license. Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or CASAC-T preferred.Benefits
