PURPOSE OF POSITION
The purpose of the Substance Use Navigator (SUN) is to work with patients primarily in the emergency department to complete chemical dependency assessments and provide linkages to care and community resources. The SUN helps patients access evidence-based treatment for Substance Use Disorders (SUDs). The Substance Use Navigator will be responsible for the opioid use disorder program and collaborate with the Clinical Pharmacy Specialist- Addiction Medicine and Complex Pain. This position will support the transitions of care and social medicine teams.
RESPONSIBILITIES & EXPECTATIONS
Assist with identification of patients with SUD. Facilitate patient engagement in treatment. Support patients and providers during ED initiation of medications for OUD and provide harm reduction supplies. Establish follow-up care navigation. Provide patient referrals to other needed services, such as mental health, primary care, and residential treatment facilities. Collaborate with department managers to meet patient expectations, achieve quality outcomes, and build a patient focused culture. Documents education, monitoring, and clinical activities in the medical record in an appropriate and succinct manner. Understands regulatory requirements and standards affecting his/her scope of practice. Enter medication incident reports in Midas and enter clinical interventions identified in Sentri7 and Quantifi software (or other current intervention tracking software used by Aultman Pharmacy. Performs other duties as assigned by the supervisor.Job Requirements
Education: Graduate of approved school with degree in social work or related field and licensure as a social worker. Licensure/Certification/Registration: current licensure in the State of Ohio Experience: Experience with Substance Use Disorder (SUD) and Medication Assisted Treatment (MAT) preferred
