Supervisor Clinical Program RN will assist patients with chronic conditions. This position will work collaboratively with physicians, staff and other health care professionals to provide evaluation and assessment of patient needs for care coordination across the health care continuum. Duties involve indirect (via phone or online) patient care activities requiring simple to complex skill sets including motivational interviewing and stages of change assessment. This position will provide customized care plans for patients.

Required qualifications:

Graduate of nursing program Texas Registered Nurse License upon hire. 2 years of experience in general nursing

Preferred qualifications:

1 year of care management experience 1 year of supervisor experience
