S:US Is seeking a Supervisor of Social Services for our transitional housing shelter in Brooklyn.

 

This position is locaterd at 1329‐1331 St. John’s Place Brooklyn, NY 11213.

 
Schedule: Monday-Friday 9a-5p, with flexibility and on-call.

The Supervisor of Social Services will lead a team of four (4) Case Managers and Housing Specialists in a 68 bed transitional setting assisting individuals with HIV/AIDS attain permanent housing.

                      

Job Functions:

Provide leadership and guidance to Case Managers around mental health and substance abuse issues, suicide prevention, psychiatric emergencies, crisis intervention, and consumer incidents.Provide administrative and clinical supervision to Case Managers, which includes but is not limited to; documented individual supervision meetings, ongoing feedback regarding job performance, participation in case conferencing and service planning, monitoring and evaluation of documentation, staff development, training, and annual performance appraisals.  Develop and maintain on site supportive services including, but not limited to:Peer support groups, relapse prevention, housing and ADL groups, and health education.Provide individual substance abuse and mental health counseling as appropriate.Provide training and consultation to Case Managers around co-occurring disorders and the evidence based practices used to address mental illness and substance abuse and to promote overall consumer wellness.Utilizes and monitors AWARDS database system, conducts internal chart reviews, and prepare for funder audits.Ensure that service delivery and documentation is in accordance with contractual agreements.Acts as a liaison to the HASA-EPU around admissions and room closures.Monitors daily rosters and provides information for the Monthly Occupancy Report.Monitors housing placements and participates in the HASA monthly reporting process.Participate in consumer intake, orientation, assessment, and service planning process and monitors consumer progression and/or regression toward service plan goals.Develop and maintain relationships with community based medical, mental health, and substance abuse service providers.Work as a member of the social service team in the development of on-site and off-site recreational, socialization, educational, and skill building activities with a view towards community inclusion and integration.Work as a member of the social service team in the development of on-site and off-site recreational, socialization, educational, and skill building activities with a view towards community inclusion and integration.Adhere to on-call protocol to facilitate crisis intervention, to address emergencies, and other situations that require immediate action.Participate in team meetings, case conferencing, division meetings, and trainings.Participate in the Services for the Underserved Quality Assurance activities such as incident documentation, management, and reportingCoordinates with Maintenance Supervisor around room/apartment/building inspections, fire drills, apartment/room repairs, heat and hot water, habitability and consumer safety issues.Reports to the Regional Director of Homeless Services in the absence of the Site Director.Working knowledge of various computer software programs including email, Microsoft Word, Excel, and PowerPoint.Evening and weekend hours may be required.Performs other duties related to the program as defined by the Site Director.

Qualifications Must be Licensed Master Social Worker (LMSW). CASAC or CASAC-T strongly preferred. Minimum of three (3) years clinical experience with individuals with HIV/AIDS, substance abuse issues, mental illness, and history of homelessness required. Minimum of two years supervisory experience required. Strong written and verbal communication skills required. Knowledge of the NYC HRA HASA entitlement system preferred.Experience in residential setting strongly preferred. Experience in group work, tenant organization, intake and assessment, co-occurring disorders, community outreach, and program development strongly preferred.
