This position provides leadership, vision, and belief in the Program of All Inclusive Care (PACE) philosophy of care; has significant responsibilities for assuring home care services are provided in compliance with PACE and State regulations. Provide care in a manner that assures participant satisfaction and retention. Organize, coordinate, direct and evaluate care functions as delegated by the Director, of Clinical Operations and Care Management. The Home Care Coordinator provides leadership Home Care Service departments for Providence ElderPlace. The Home Care Coordinator provides supervision to the Home Care workers. Provides oversight and management of services received in participant's homes to assist them in remaining as independent as possible.

Required qualifications:

Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 4 years of experience in coordinating, managing, monitoring, and providing community social/health services. 2 years of Supervisory experience

Preferred qualifications:

Associate's or Bachelor’s Degree in social work, nursing, geriatrics.
