The Admissions and Outreach Supervisor will have accountability for managing all aspects of the admissions, in house transfers and discharge process for the Skilled Nursing and Sub Acute programs. To be successful, the Supervisor must work collaboratively with hospitals, skilled nursing facilities, referral agencies, discharge planners, social workers, and in- house staff to achieve budgeted census levels. As necessary, directs admitting staff to coordinate quality admission service.

Bachelor's Degree in Social/Human Services, Business, Marketing or Nursing (OR five years experience in health care setting with emphasis in gerontology may be substituted for the educational requirement).2 years experience in long term care, clinical case management and admissions.2 years experience in case management and contract management, along with experience in information systems, billing and analysis2 years or more years experience in a marketing and/or sales environment.
