Major Duties: Supervisory and Managerial Services:- 50% Provides direct supervision for the Primary Care department including all functions of supervising, planning, organizing and developing primary care ambulatory medical services . Confers and make recommendations to the Clinical Director and Service Unit administration in representation of the Medical Staff pertaining to policies, initiating of new therapies or need for new medical services and equipment. Maintains medical staff credentialing files for all providers within the direct report departments and ensures monthly peer reviews are being completed. Direct Patient Care Services-50%: Diagnosis, prevention, therapy, maintenance and rehabilitation of patients. Provides independent diagnostic, preventive and medical services for patients of varying complexity and advanced nature utilizing advanced clinical and analytical skills. Maintains a process for systematic follow-up by; determining the effectiveness of the treatment plan with documentation of patient care outcomes, reassigns and modifies the plan with the patient and family as necessary to achieve health and medial goals. Completes comprehensive evaluation, management, and follow-up of chronic and acute conditions in the ambulatory care setting, including complex conditions. Provides health education and treatment plans to patients and families. Completes patient charting and patient referrals thru electronic health records ( EHR).
