Seeking a Supportive Housing Case Manager!

Responsibilities include working to engage potential and current program members to establish a positive working relationship which promotes ongoing improvement of said relationship, as well as recovery. This will be done through monitoring mental health status, recovery, progress, readiness, recovery promotion, and taking steps to reduce the need for hospitalization.

They will also help to link program members with appropriate crisis management services. Case Managers may also be asked to provide transportation for program members to and from appointments when no other transportation is available.

Applicants must have a minimum of an Associate’s Degree in a Human Services Related field with one year of related experience, although a Bachelor’s Degree is preferred.
A valid NYS driver’s license is needed, as travel within the community is required.

Starting salary for this position ranges from $17.91 to $19.70, based on experience.
