Supports Coordinator Assistant Job Details Job Location Livonia Clinic - Livonia, MI Education Level High School POSITION SUMMARY: The support coordinator assistant is responsible for the assessment, referrals, linking and coordination of all services, as well as for monitoring and prescribed services under the supervision of a support coordinator. The support coordination assistant is the responsible party for the clinical record. EDUCATION AND / OR EXPERIENCE: + Minimum of a High School Diploma is required. An Associate’s degree in mental health or related human service field is preferred. + Must have the ability to receive QMRP status through Wayne County and agency training. + Experience in working with and assessing children according to their individual needs. + Experience in assessment of children according to developmental level. + Experience working with developmentally delayed, mentally ill, or disabled children and their families is required. + Experience with ages 0-18 preferred. + Experience working in the home and/or community setting preferred. DUTIES AND RESPONSIBILITIES: + Responsible for the coordination of all supports regarding children’s services for developmentally disabled children ages 0-18. + Assesses the needs of the family and begins the process of person-centered planning. + Provides all referrals, linking and coordination of services for families in the Developmental Disabilities network. + Maintain caseload size and weekly productivity expectations as determined by the supervisor. + Maintain clinical charts in an accurate and timely manner to include, but not limited to assessments, treatment plans, and all other MDCH chart requirements. + Maintains responsibility for the clinical record. + Through direct contact and correspondence, ensure Starfish Family Services has a positive relationship with all referral sources and related agencies. + Follow and comply with clinic policies/procedures. + Must meet the productivity expectations as assigned by program supervisor. + Adhere to all policies and procedures as it relates to documentation, training requirements, CMH guidelines and confidentiality. + Participate in agency meetings, training and other agency functions as needed or requested. + Attend departmental and agency meetings and trainings as required. + Assist in finding and directing clients in case of an emergency such as a tornado or fire. + Perform other reasonably related tasks assigned by management or required by the team. KNOWLEDGE, SKILLS, AND ABILITIES: + A range of knowledge in mental health topics, techniques, theory and service delivery with children and families 0-18 required as well as service systems and levels of care. + Demonstrate empathy and compassion necessary to facilitating positive growth. + Demonstrated knowledge of child, adolescent and/or adult developmental stages and signs of conflict or distress. + Competency, or willingness to learn, co-occurring treatment of mental health, substance abuse and/or developmental disabilities. + Development and implementation of Individualized Plan of Service utilizing Person Centered Planning. + Competency, or willingness to learn, a wide range of treatment modalities that are individualized and person centered. + Ability to develop treatment plans and coordinate needed services to fulfill plans. + Demonstrated ability to set and meet program outcomes and objectives. + Ability to communicate effectively both orally and written. + Ability to plan and organize time effectively. + Ability to present self professionally to staff, community, other social service agencies and referral sources. + Ability to solve problems and make decisions, especially during crises. + Must have a working knowledge of all SFS programs within the first 90 days of employment. + Ability to manage multiple priorities, function independently and demonstrate good follow-through. + Ability to work positively, cooperatively and productively as a member of a team with colleagues, supervisors and agency staff. + Computer literacy, including PowerPoint and excel. + Cultural sensitivity/competence working with multicultural client populations + Ability to maintain ethical and professional standards. + Must respect the confidentiality of all program participants. + Interest and willingness to continue personal and professional growth and skill development. + Ability to work with all people regardless of race, gender, religion, age or ethnic background. LICENSING AND OTHER REQUIREMENTS: + Must have a valid State of Michigan Driver’s License and automobile insurance that is insurable by our carrier.
