Short Description
Monitors day-to-day task completion by LPNs
Creates and updates LPN schedules to ensure coverage at all sites
Oversees phone triage and EPIC In-Basket work collaborating with providers, medical residents, LPNs and medical assistants
Oversees nurse visits and nurse protocol visit, including the ability to see patients on schedule as needed
Addresses patient related concerns impacting patient care services such as urgent patient issues, prior authorizations, unanswered messages and nurse visits
Provides quality assurance for clinical messages and nursing notes
Participates in process-improvement activities related to phone triage, clinical message taking and follow-up, nurse visits and LPN protocol visits.
Collaborates with Director of Clinical Practice to facilitate resolution of issues related to LPN clinical functions
Completes monthly rounding on LPN staff
Completes 90-Day and Annual Evaluations on LPN staff
Performs all related duties or special projects as assigned/required
Experience Required
5+ years recent clinical experience as LPN, preferably in an ambulatory practice
2 years + of supervisory experience preferredEducation Requirements
High School Degree/ Associate Degree Preferred
License/Certification Requirements
LPN License Required and BLS through the American Heart Association
