patients and the medical community offering primary care urgent care and more than different specialtiesJob DescriptionThe Surgical Clinical Reviewerrsquos SCR chief responsibility is to collect and submit reliable data to the ACHQC Abdominal Core Health Quality Collaborative and other appropriate hernia registries This task is accomplished through high quality data compilation documentation and entry of hernia procedures performed at the center The SCR works closely with the members of the hernia and general surgery departments to identify opportunities for clinical quality improvement and other special projects as may be identified The SCR shall have access to all relevant patient documentationreporting systems to ensure data collection This position works in collaboration with the surgeons clinical teams and Quality Departmentmiddot Identifies collects and reports hernia surgery data accurately and timely for multiple time intervals over the course of the patientrsquos care through effective utilization of the hospital and surgeonsrsquo office record systemsmiddot Works in collaboration with both clinical and administrative staff across the program to ensure accurate data collection and data reporting middot Represents the hernia program at identified meetings to ensure reliable data collection and reporting including preparing and presenting hernia data reports middot Identifies streamlining and process improvement opportunities in the data collection process and quality of patient care middot Establishes effective working relationships with members of the hospital community and surgeon offices especially staff in the surgery medicine nursing medical records information systems departments and others whose support is necessary for the management and success for the program middot Serves as an educational resource on the Hernia Data Registry Platform for internal and external audiences by developing educational material and delivering presentations middot Participates in hernia data registry activities such as program teleconferences and program meetingsmiddot Participates in data compilation for research projects audits and other center of excellence applications ie private payer if applicablemiddot Supports the HHC hernia data registry by auditing the registrys cases monthly to ensure alignment and data accuracy with ACHQC acceptableExperiencebull Minimum of years medicalsurgical hospital or office nursing experience or experience in clinical researchmedical records or clinical quality improvementbull Clinical knowledge and understanding of patient care related to data collection and identification of areas for quality improvementbull The ability to learn and utilize new software and web applications for data entry and report generation Competence with Microsoft Officereg Word Excel PowerPoint and basic statistical skills The ability to learn Microsoft Accessbull Competence with Epic EMR documentationbull Experience gathering information in a complex hospital system environment bull Highly organized with demonstrated ability to effectively prioritize competing tasks with full autonomybull Excellent analytical and problem solving skills bull Able to articulate clearly and apply sound judgment bull Ability to travel independently to satellite offices with or without advanced noticeWe take great
