Survivorship Coordinator

Job Summary

Coordinate and participate in the planning, organizing, supervising, implementing, and evaluation of survivorship and cancer patient navigation services. Coordinate with Clinical Practice Administrator, medical staff, professional nursing staff, other departments, and community agencies. Demonstrate proficient and advanced clinical skills in planning, implementing, and delivering care to cancer patients.

 

Responsibilities

• The Survivorship Coordinator will play an active role to insure the CoC Survivorship Standard meets the accreditation requirements.

• The Survivorship Coordinator will work with the registry, the practices, outpatient chemo, etc. to identify patients that should receive the survivorship care plan and have a survivorship visit with a cancer provider that was involved in her/his care.

• The program coordinator conducts patient studies to evaluate activities and outcomes, manages policies and protocols, and reviews current best practice through use of nationally recognized guidelines and ongoing education. The coordinator may recommend, develop and coordinate improvement activities.

• Manages and coordinates patient care in area of specialty within the program and with resources outside the program.

• Collects and analyzes on-going data regarding outcomes the Program. Submits required data to clinical manager as requested (examples: patient satisfaction, , clinical outcome, present findings to hospital committees as required).

• Serves as Consultant to intra-professional staff in area of specialization.

• Maintains current knowledge and competency in the area of specialty and in professional practice (i.e., attends conferences and workshops, participates in professional organizations, Evaluates and disseminates research findings, application of research findings to practice, and participation in nursing research,

• Establishes and maintains Quality Standards (i.e., develops and implements policies and procedures, monitors accreditation standards, and maintains compliance with all hospital, county and state regulations, maintains all required documentation and reports).

• Communicates effectively with patients, families, staff, and physicians (i.e., displays sensitivity and understanding of cultural groups; always exhibits strict patient confidentiality).

• Demonstrates computer skills necessary to enhance job efficiency (i.e., proficient in use of database and program, word processing, error-free report generation, learns and operates current patient care management systems).

• Maintains clinical and/or professional competency (i.e., maintains licensure and/or certification, provides patient care or professional support in those areas for which evidence of competency can be provided.).

• The Survivorship Coordinator will be responsible for advancing patient education, improving patient and provider communication, enhancing satisfaction, and promoting better utilization of clinical services. This individual will be responsible for the coordination of the administrative, operational, and patient navigation functions of the Program. The goal is to ensure patients receive appropriate services, support, and education throughout their cancer care continuum. The goal is to ensure patients effectively and efficiently transition through the continuum of breast cancer care from newly diagnosed through long-term survivorship receiving appropriate services, support, and education.



Leads training programs for internal and external
professionals working with cancer population.



• Assesses potential areas for survivorship and navigation programs/services, identifies resources available to meet the needs of patients and the community, develops and plans survivorship and navigation programs, directs the implementation of programs, ensures evaluation of the programs, and disseminates results of the evaluation and makes recommendations based on the results.

• Develop, monitor and evaluate the effectiveness of the navigation, survivorship and public education programs

• Recruits, trains, orients, motivates, schedules, evaluates and supervises health educators, research assistants, residents, student interns, and volunteers; assign and review work activities to meet productivity goals, conduct performance evaluations and implement quality improvement efforts.

• Prepare summaries, presentations, and reports for administrators

• Other duties as assigned.

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Education 

• Bachelor's Degree (Required) Education: Bachelor's Degree in Nursing OR actively pursuing a BSN Experience: 3 Years Oncology Experience Substitutions: 36 Hours of continuing oncology CEU every 3 years a substitute for the Oncology Certification. Comments: Must complete and be credentialed as a Nurse Practitioner within 15 months of hire.
