The Western York Region Ontario Health Team (WYR OHT) is a partnership of local health care providers who are committed to better health and experiences for patients and providers in Vaughan, Richmond Hill, King and surrounding areas. The System Planner will report to the Director of the WYR OHT and be accountable to the WYR OHT Collaboration Council. As a key resource to the OHT, the System Planner will build and maintain strong relationships with internal and external partners and will employ leadership and best practices to plan and implement the OHT. In this role, you will be an innovative leader who excels in "engaging people fully", with planning and decision-making that enables a culture of collaboration amongst partners. The OHT System Planner will be accountable for supporting initiatives to advance integrated care and partnerships across a variety of sectors including Primary Care, Acute Care, Home and Community Care, Public Health and Community Support Services.
Responsibilities:
Project Management and Administrative
Applying project management methodologies to plan and manage projectsDevelop and coordinate the distribution of project management related materials, including project plan, work breakdown structures, milestone maps, status reports, etc Lead, organize and facilitate project-related meetings, develop project reports, presentations, statistics and other related documents, as requiredContinuously evaluate and assess each project milestone closely to identify lessons learned and best practices for implementation in future phasesEnsure project deliverables are met on time, within budget and within scopeDevelop and implement required project management tools to support OHT membersManage interdependent planning work streams, timelines and deliverablesProvide mentorship and support to Project Managers and Communications and CommunityEngagement SpecialistRelationship Management and Engagement
Support the development of integrated collaborative care and partnership opportunitiesContributes to and implement patient, caregiver, provider and community planning and engagement strategiesManage and maintain stakeholder maps, lists and contacts
Communication
Prepares and submits grant/funding applications that advance integrated collaborative care, coordinates reporting to the Ministry of Health (MOH), Ontario Health or other groups, and supports the evaluation of integrated care initiativesContribute to safe, quality care by
Managing a workload effectively in a fast-paced environment.Addressing and resolving issues in a productive, respectful manner.Exercising sound professional judgment and using best practice guidelines. Contributing to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.What must you have?
Master’s degree in Public, Business or Healthcare Administration or equivalent experience requiredMinimum 1-3 years healthcare or public sector experience requiredRegistration and membership in good standing with a regulated health professional college preferredPMP designation preferredWhat else do you bring?
