TARGETED CASE MANAGER

The Targeted Case Manager’s purpose is to identify the medical, social, educational and other needs of the eligible family, identify the services necessary to meet those needs, and facilitate and coordinate access to those services. The Targeted Case Manager is responsible for the overall management of assigned caseloads of children/families served by KidsPeace New England.
QUALIFICATIONS:
Level I Case Managers: Meets the qualifications for Level II Case Manager OR be any agency staff who has parented a child or adolescent with special needs and is able to meet specified staff, training and supervision requirements. Approval from BDS Children's Services must be maintained in the personnel file. Level II Case Managers: a. A Bachelors Degree from an accredited four (4) year institution of higher learning with a specialization in Psychology, mental health and human services, behavioral health, behavioral sciences, social work, child development, special education, counseling, rehabilitation, sociology, nursing or closely related field; OR b. Be a graduate of an accredited graduate school with a Master’s Degree in social work, education, psychology, counseling, or closely related field; OR c. Be a Registered Nurse with at least two years of full-time equivalent psychiatric experience with children or adolescents; OR d. Have a Bachelor’s Degree from an accredited four (4) year institution of higher learning in an unrelated field and at least one year of full-time equivalent relevant human services experience.JOB DUTIES:
To provide intake, assessment, plan of care development, coordination and advocacy, monitoring, and evaluation services. Regularly schedule meetings with assigned child and family on caseload to provide support, resource information, guidance, and advocacy. Coordinate, monitor, and act as liaison between necessary clinical, educational, behavioral, and all healthcare services with KidsPeace professionals or community-based providers in accordance with KidsPeace policy as well as external licensing and regulatory bodies. Maintain documentation for each client file in compliance with KidsPeace Quality Assurance standards as well as external licensing and regulatory bodies. To communicate regularly with all members of the treatment team, and other collateral contacts, regarding routine and emergent issues in accordance with KidsPeace Quality Assurance standards as well as external regulatory and licensing bodies.
