The Ambulatory Care Pharmacist participates with other team members in the primary care setting to manage our patient’s health. He/she is primarily responsible for developing and implementing pharmacy related processes and procedures within the Christ Hospital Hematology and Oncology Offices that support improved health outcomes for defined populations. He/she will assist providers in developing effective medication regimens, provide assistance for patients with polypharmacy issues and review patients with high risk medication profiles.

This position is focused in oncology and primarily supports our outpatient oncology practices and their oral oncology counseling program.
