Team Lead, RN Case Management
Summary The Team Lead-Care Management role coordinates daily activities of care coordination colleagues and advises on priority projects by consulting with manager. Provides general administrative direction over patient education related to illness and planned treatment, medication management in collaboration with the primary care provider, and promotes patient self-management. Works in partnership with entire healthcare team to manage transitions of care and close gaps in care. Provides oversight in standardizing processes and procedures related to patient care and medical management. Qualifications Education Associate’s Degree or Diploma in Nursing is required. Bachelor’s Degree in Nursing is preferred. Experience 3 years nursing experience with at least 2 years nursing experience in care management, home health or related experience is required Quality/Process Improvement experience is preferred. Certifications, Licenses and Registrations License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules. Case Management certification (ACM, CCM, RN-BC) is preferred. Basic Life Support Certification (BLS) is required within 60 days of hire.
