+ JOB DESCRIPTION DETAILS + Job Summary : + Collaborates with patients, families, advocates, healthcare providers, and community resource representatives to promote health and communicates available community support resources to at-risk populations presenting to the healthcare system. Makes patient assignments to SW team for L5 and L4 and fulfills programming assignments + Education : + Position requires a Master's Degree in Social Work from an accredited college or university. + Registration/Certification/Licensure : + Position requires licensure in the state of Maryland - LMSW, LCSW-C, LCSW, LGPC or LCPC. + Experience : + Position prefers 3 -5 years related medical experience in an acute care setting with one year of clinical supervision, and prefers relevant experience in Behavioral Health. + Other Requirements : + Maintains unit-specific and hospital competencies, mandatory learning, and any clinical certifications required in accordance with the Staff Education and Training policy GA-057 and/or any other department requirements. + Effectively communicates with all patient populations and staff by implementing organization-wide communication techniques as a daily practice. + Effectively advocates for quality patient safety and satisfaction as a daily practice. + Demonstrated proficiency of computer skills necessary to effectively complete position requirements. + Interviewing and screening SW candidates + Previous experience and demonstrated professionalism in effectively communicating with customers and in promoting a positive service atmosphere. + Ability to work independently in a multi-task environment producing quality work.
