Summary:

The 3 Tele Team Leader is responsible for assessing the needs of each patient ranging from adolescent to geriatric and planning the nursing care necessary to meet those needs, including the spiritual, emotional, psychosocial and physiological needs. The Team Leader is then responsible in seeing that the interdisciplinary plan of care is carried out through performance or delegation to subordinate personnel. The Team Leader is supervised by the Director and Unit Manager.

 

Experience/Education Requirements:

BSN required within 4 years of hire, Master's degree in nursing preferredMiniumum of 3 yrs cardiac experience

License or Certification Requirements:

Current Illinois RN registrationCurrent CPR & ACLS certificationCertification as CCRN within 1 year of hire

 

 

Responsibilities:

Able to troubleshoot equipment and problems/failures.Acts as a liaison between staff members and physicians.Assumes a patient assignment as needed.Communicates with and informs physician or nursing house supervisor of any information pertaining to patient care.Completes staffing worksheet taking a brief report on patients that are in unit.Consistently aware of all actual/potential problems/conditions.Cooperates with other departments to provide interdisciplinary coordination.Coordinates patient care and unit activities.Ensures that patient assignments are appropriate for patient needs and staff qualifications.Manages environment on the unit.Offers assistance to physicians rounding on the unit.Offers assistance to staff as needed.Participates in quality assessment/improvement activities on unit.Patient rounding with inter-disciplinary care team.Performs daily staffing with nursing office or nursing supervisor.Responsible for checking crash cart seal.Satisfactorily complete all required continuing education hours regarding the care of acute stroke patients.Works closely with other units when patient census warrants acceptance of alternate admissions or need for immediate transfers.

Blood and Body Fluids: Diagnostic composite of patients, frequent contact with intravenous devices, and high potential for contact with contaminated body fluids (blood, stool, vomitus) and administration of blood does create a high risk. use of needleless IV system, universal precautions and protective equipment minimize risk.
TB or Airborne Pathogens: Patient diagnostic presentations do pose high risk; however usage of protective wear and universal precautions reduces risk.

Sensory requirements (speech, vision, smell, hearing, touch):
Speech: Required to communicate with patient/families/staff/MD's. Ability to ask for help.
Vision: Required to observe patients and their physical assessment, ability to read reports, ability to read machines.
Smell: Helpful to note presence of incontinence, vomitus, blood; electrical/fire safety.
Hearing: Necessary to hear alarms, calls for help and to hear patient verbalizations; ability to listen for lung, bowel, heart sounds.
Touch: Necessary to assess patients, need to distinguish between hot and cold.

Lifting of patients off their beds that are unable to move independently is done with assistive lifting equipment. Horizontal movement of patients to and from carts/beds who are unable to move independently is done with assistive lifting equipment. Lifting equipment - carry 25'-150' waist high (IV bags, patient trays). Carry charts 25'-150' waist high. Filled laundry bags are filled half full so as not to weigh over 35 lbs. Carry fire extinguishers up or down 3-4 flights of stairs and several yards in an emergency.
