Schedule: Mon - Fri 8am - 4pm or 4 days 8am - 6pm; This position will also require flexibility based on program needs and weekend hours on a rotational basis

Salary: $85,000 / year

Sign On Bonus: $5000; $1666 upon completion of 30 days, $1667 upon completion of 6 months, $1667 upon completion of 1 year

We are seeking a new PACT Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness. The PACT Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services. The PACT Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.

Other responsibilities include but are not limited to:

Provide case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment team Conduct a comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist persons served with activities of daily living.
