Adult Full Service Partnership (FSP) and Laura's Law/Assisted Outpatient Treatment (AOT) program serving a total of 240 adults with serious mental illness and may currently be experiencing or be at risk of homelessness. Members referred may qualify for Permanent Supportive Housing through No Place Like Home (NPLH) units.
Shifts Available:
Full Time; AM 8:30 am - 5:00 pm varies as needed; Monday - Friday
Expected starting wage range is $66,560.00 - $96,013.10. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
POSITION SUMMARY
Under direct supervision of the Clinical Director/Administrator, this position facilitates a multidisciplinary team, coordinates service needs, collaborates with other services and agencies, and directly supervises assigned team members. Fulfills appropriate level of services for new members served as outlined in the essential functions
QUALIFICATIONS
Required:
· Bachelor’s Degree and (3) years’ experience in a mental health setting and at least two years supervisory experience in a healthcare setting.
· Completion of supervisor training within (6) month of hire
· Sensitivity to multi-cultural populations and issues
· Must be at least 18 years of age
· Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
· All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Position requires driving, valid driver’s license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply
Preferred:
· Outpatient clinic or community mental health service with 2 years of direct clinical experience
· Experience providing intensive case management and/or intensive community supports using psychiatric rehabilitation concepts is highly desirable
ESSENTIAL FUNCTIONS
· Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders
· Functions as scheduling coordinator and; ensures coordination of administrative team functions including scheduling of staff and members served
· Directs the day-to-day clinical/administrative operations of assigned team, ensuring all members served receive services as indicated in their treatment plan
o Assigns staff to work with individual clients based on their individual needs and the individual skill sets of staff members
o Directs staff in the performance of their job duties
o Trains and coaches staff
o Evaluates job performance
o Participates in disciplinary process as needed for performance issues
· Participates in the recruitment and selection of new staff members
· Oversees and monitors accuracy of medical records
· Safeguards members served confidentiality
· Promotes a high involvement culture by aligning the team around common objectives in order to ensure contract outcomes, ACT fidelity, and organizational standards are met and maintained
· Ensures staff documentation, assessments, and paperwork meet federal, state, county, and organizational standards and timelines per contract requirements
· Engages, inspires, and facilitates meaningful conversations with members served that assist the person to explore, create, and meet their own recovery goals
· Responds to critical situations with a high-level of engagement and de-escalation skills that support a least restrictive environment for individuals experiencing an emergency related to a mental health/addiction challenge
· Organizes team meetings to update and coordinate assigned tasks and work flow for team members
· Ensures required deliverables are met or exceeded by self and team members
· Ensures accuracy of records and safeguards client confidentiality
· Monitors documentation and other required standards
· Collaboratively works with internal and external customers
· Provides on-site recovery and skill coaching and development
· Monitors and supports staff attendance, required credentials, and performance development of team members
· Travels locally to provide outreach to members served in the community (program specific)
· Understands payer documentation requirements and can supervise others to meet these requirements
· Develops, updates, and implements individual problem list
· Abides by established policies, procedures, and standards of care in accordance with published ACT Standards
· Tracks, maintains, and ensures compliance with contract expectations and outcomes within scope of position
· Establishes strong customer and community relations
· May be assigned to take calls from team members 24/7 regarding client care
Position requires driving own personal vehicle for company business such as trainings, offsite meetings, etc.
SKILLS
· Ability to work effectively as a member of a multidisciplinary team
· Ability to train, empower, and lead staff to embody above elements in daily interactions with colleagues, members served, and other service providers
· Thorough understanding of the rights of members served and related legal processes
· Effective written and verbal communication
· Ability to assess safety risks
· Excellent driving skills
· Within three (3) months of employment, ability to apply financial requirements for billing and learn billing codes
· Ability to audit progress notes and treatment plan for payer compliance
· Familiarity with stages of change and motivational interviewing

The employee is occasionally required to reach, twist, bend, squat, kneel and lift and carry items weighing 25 pounds or less as well as to frequently sit, stand, walk, push, pull and do simple and power grasping and to occasionally drive.
