Overview

The Transitional Care Tele Triage LPN provides services to the assigned population in accordance with protocols. The transitional primary care LPN serves as a resource for telehealth patients and providers. You will serve as a coordinator for this virtual visit program.

Responsibilities Schedule needed virtual visits in coordination with telehealth providerReceive and upload records into HCHB, starting client chartSend RingCentral link to designated HH clinician and client for scheduled visitEnsure compliance with state/local requirements through proper documentationParticipate in performance improvement activities and maintain ongoing clinical knowledgeDocument in real timeActively participate in applicable meetings as requestedMay complete SNF Triage LPN duties as needed Qualifications 2 years of experience in a clinical healthcare setting High school diploma/ GED required; Associate’s Degree or some college preferredLicensed Practical Nurse (LPN) with an active license; compact license preferred.Experience with electronic medical record software and proficient use of the Microsoft Office suiteAbility to multitask, prioritize, and manage multiple projects simultaneouslyExcellent written and verbal communication skills and ability to work independently or in a team setting About our Line of Business Starting Point Nursing Services is staffed 100% by a team of remote Registered Nurses trained to support the Post-Acute Continuum with clinical advice and interventions. Our services are tailored to our partners, including remote physical/chart assessments, predictive care modeling, clinical programming interventions for high-risk populations, emergent care coordination, and case management.
