(Temp) Substance Abuse Counselor
Overview:  Vitalief is a leading site solutions partner focused on improving the value and contribution of clinical trial sites across the drug development lifecycle.  We offer integrated Consulting (strategic and operational), Functional Service Provider (FSP) services, Workforce Solutions (training and career pathing), and Site Networks that enhance clinical trials and improve patient outcomes.  We are seeking a Licensed Social Worker to temporarily join our exceptional client team to change patient lives and outcomes.    WHY VITALIEF?   + You can actively contribute to our clients’ mission of advancing scientific discoveries that have the potential to change patients' lives for the better.  + Our PEOPLE FIRST culture prioritizes personal and professional growth for all Vitalief employees.  + We give everyone a seat at the table – we encourage innovation.  + Life/work balance that includes 20 PTO (Paid Time Off) days plus 9 paid Holidays annually.  + Other benefits include Company paid life insurance and short / long term disability coverage; 401K retirement program; Robust healthcare plans to choose from.  Salary Range: Market Competitive; Based on experience level   Work Location: Hybrid; 4 days on site in Newark, NJ.   Responsibilities:  + Managing the daily workflow in order to achieve high quality care management delivery and positive clinical outcomes  + Assessing the needs of a patient by working with patients, medical providers, family and community support service providers   + Conducting patients health and psychosocial risk assessment in order to implement a plan of care based on their needs   + Develop and implement the multi-disciplinary plan of care for patients with complex mental health and/or substance abuse and socio-economic concerns   + Maintains updated documentation of reviews, including, but not limited to diagnosis, symptoms, interventions, treatment goals and plans  + Monitors and coordinates external outpatient, ancillary and primary care services.   + Collaborates with local community services to establish linkage opportunities to ensure appropriate access to services   + Maintains and provides statistics on volume and services as needed for reporting and evaluation   + Participates in quarterly client care reviews and monthly high risk care coordination meetings   + Keeps abreast of all pertinent federal, state and client regulations, laws and policies as they presently exist and as they change or are modified.  Required Skills:  + Minimum of 2 years of field experience in mental health or substance use   + Minimum of 1 year working in managed care and or case management   + Masters Degre e in Social Work, Counseling or a related field is a must   + Experience in a clinical setting caring for patients with HIV/AIDS a plus   + Bilingual in Spanish a plus   + Excellent verbal and written communication, presentation and interpersonal skills.  + Ability to effectively present information and respond to questions from managers, clients, and patients.  + Knowledge of ICD10 and DSM-V coding is a plus.        PHYSICAL DEMANDS: Prolonged periods of sitting at a desk and working on a computer.  Standing, walking, visual perception, talking and hearing.  Lifting up to 20lbs.    Powered by JazzHR
