TFCO FAMILY CLINICIAN Job Details Job Location Herkimer Road Office - Utica, NY Position Type Full Time Education Level Graduate Degree Salary Range $67,376.00 - $77,376.00 Salary Travel Percentage Negligible Job Shift Day Description Basic Function/Overall Responsibilities: Ensure the development and implementation of an individualized, trauma informed and holistic treatment plan for all families on assigned caseload, maximizing participation of the child and key family members. Provide clinical services specific to the child and family’s needs to accomplish identified discharge/reunification outcomes while working collaboratively through initial and ongoing assessments. This will be guided by the Treatment Foster Care Oregon (TFCO) model of intervention with a specific focus on providing support and advocacy to the child’s parent/guardian/”forever family” in an effort to support the return of their child utilizing the prescribed behavior management approaches. Principal Assigned Responsibilities: + Provide weekly family therapy to caretakers of TFCO children (parent/guardian/”forever family”) using the TFCO treatment model. This includes introducing new behavioral, coping, and parenting skills and practicing these in sessions using strategic positive reinforcement and coaching interventions. + Provide post-discharge support to families with children who have returned to their home for permanency. + Participate in 24 hour on-call availability for program parents, The Treatment Foster Care Oregon Family Clinician will share responsibility with the Treatment Foster Care Oregon Family Clinician Team Leader for 24/7 on-call availability. + Coordinate treatment efforts and interventions with the TFCO Individual Clinician on the team. + Participate as the co-lead clinical member of the weekly TFCO Team Meetings specific to each family on assigned caseload. Ensure the development and implementation of a comprehensive, individualized and discharge/reunification focused treatment plan, utilizing a strength-based approach. Monitor implementation of treatment plans and revise as necessary. + If applicable, collaborate with the family’s overall service team as it pertains to the stabilization and success of the foster care child. This may extend beyond the TFCO Team, including, but not limited to, community/mental health providers. + Provide initial and ongoing assessments of families receiving services. + Attend additional meetings as deemed necessary by the treatment needs of assigned caseload. + Provide record keeping and documentation for all clinical activities (case documentation). + Ensure that the frequency, nature, and location of clinical contacts follow agency policy and regulatory requirements. + Maintain required documentation associated with the provision of services in a timely and accurate manner via the agency’s Electronic Health Record (EHR) and OCFS Connections System as required by relevant regulatory bodies. Clinical documentation includes, but is not limited to, the following: + Psychosocial assessment and/or semi-annual assessment updates. + Individualized, comprehensive treatment plans that accurately reflect specific discharge criteria, client-centered goals, measurable objectives, child/family participation in the development process, and modalities/methods designed to help the child/family meet the criteria for discharge, and periodic progress notes relevant to goals and objectives on client treatment plan. + Treatment progress documentation as required by agency’s standards and relevant regulatory bodies. + Other documentation associated with the provision of services (e.g. incident reports, court reports) + Partner with staff from other disciplines and /or Foster Parents as relevant to assist in ongoing program evaluation and development and staff training in treatment-related skill areas. 12. Provide crisis management and problem-solving support services for caseload within the context of the child's home, school and/or agency program as deemed necessary. 14. Successful completion of all training as defined by program and/or agency policy. Provision of training as requested by supervisor and the agency Training Department. 15. Maintain and continually develop clinical skills and competencies by actively participating in regular individual and group clinical supervision, agency trainings and professional workshops or conferences as defined by program and agency competency standards. 16. Participation in program and agency-wide performance improvement activities as requested. 17. Other associated tasks as assigned by supervisor.
