The Therapist provides comprehensive BioPsychoSocial assessment to consumers within 10 business days of referral to services. Based on BioPsychoSocial and screening tools and collateral from judiciary personnel and staff, a treatment plan in conjunction with the consumer is created to address clinical and SDOH needs. Carries a caseload of consumers doing individual therapy. Provides individual and family counseling to consumers pending linkage to outpatient services. Maintains treatment records. Excellent communication and interpersonal skills. Can identify and implement trauma-informed therapeutic techniques for individuals, families, and groups. Can administer and interpret assessment instruments utilized by the program. Demonstrates the knowledge and skills necessary to provide care appropriate to the consumers' age-related needs. He or she must be able to assess and interpret data about the consumer's status to identify each consumer's needs and provide the appropriate care, including age-related care, needed by the consumer group.

The Diversion Support Services program works with individuals who are incarcerated in Middlesex County and released under bail reform to link them to services, including housing, financial assistance, physical health, and mental health services. The program uses a peer-focused and strengths-based model to transform consumers from surviving to thriving.

The ideal candidate will provide individual therapy services, maintain a caseload of up to 20 consumers, and collaborate with a multidisciplinary team to ensure the delivery of high-quality mental health care.

 

Key Responsibilities:

Provide individual therapy to consumers, tailoring treatment approaches to meet their unique needs and goals. Manage and maintain a caseload of up to 20 consumers, ensuring timely and effective service delivery. Develop, review, and update treatment plans in collaboration with consumers and other stakeholders as needed. Conduct assessments and evaluations to determine treatment needs and goals. Utilize evidence-based practices such as CBT, Motivational Interviewing, and other relevant therapeutic modalities. Maintain accurate and timely documentation in compliance with organizational, state, and federal guidelines. Participate in team meetings, supervision, and professional development opportunities. Coordinate appropriate care with other providers, community resources, and support systems. Adhere to ethical guidelines and standards of practice as established by professional licensing boards and organizational policies.

 

Hours:

This position is Full Time, Monday – Friday, with some evening hours required.
