**Job Summary and Responsibilities** + Evaluates and assesses the bio-psycho-social needs of patients. + Assists in determining the most appropriate level of care for the patient, and makes appropriate referrals when indicated. + Develops comprehensive treatment plans in accordance with policy and procedures. **Job Requirements** + Prefer three years of post license experience in a mental health/addiction recovery or related field. Certification and experience with multi-occurring disorders a plus. + Minimum of a Masters degree from an accredited educational institution in Counseling, Social Work, Nursing or other related behavioral health field is required. + IA: Licensed Mental Health Counselor (Temporary accepted) or Licensed Independent Social Worker (Temporary accepted) **Where You'll Work** From primary to specialty care, as well as walk-in and virtual services, CHI Health Clinic delivers more options and better access so you can spend time on what matters: being healthy.
