POSITION SUMMARY
Reports To: Unit Director/Clinical Supervisor
Supervises: PACT Team
Training: CPI within 90 days of hire or transfer , Annual, six (6) hours in discipline, Effective 1/21 BLS required, existing incumbents with Heart Saver First Aid CPR/AED, will be required to renew with BLS, all new hires effective this same date, are required to have BLS within 90 days from hire or transfer.

Position Summary:
The Team Leader/Facilitator provides leadership, direction and coordination to the team within regulatory requirements. The Team Leader also fulfills a role of Therapist on the team
Duties and Responsibilities:
· Maintains communication with the PACT Director around team functioning and service delivery
· Empowers the team by modeling strong leadership and conveying the philosophy and principles of PACT
· Creates a climate that supports the dynamic interaction and participatory process of the team
· Ensures equal distribution of team responsibilities
· Keeps the team focused to complete daily and team issues meetings efficiently and effectively
· Promotes cross-training/education among various disciplines on the team
· Facilitates a productive decision-making process around consumer needs
· Assists consumers in creating WRAPs' and MH Advance Directives
· Provides competent therapeutic services, within scope of demonstrated competence
· Teaches and models Wellness and Recovery principles and Psychiatric Rehabilitation clinical interventions
· Completes biopsychosocial assessments and prepares a comprehensive treatment plan for all new admissions
· Engages support systems and referral sources in assessment process
· Demonstrates the knowledge and skills necessary to provide services appropriate to any age/disability group assigned.
· Assesses client for an Initial Treatment Plan and appropriately re-assesses the client's needs throughout treatment
· Completes Master Treatment Plans that are behaviorally specific and with goals and objectives that are related to client problem areas.
· Provides therapeutic services, within scope of demonstrated competence, as required.
· Provides competent crisis intervention
· Assists with case management and interagency coordination of services for clients. Makes appropriate referrals
· Valid NJ Drivers License
· Documentation must be completed within one (1) calendar day of service delivery
· Productivity goals shall meet % as designated by department
Minimal Qualifications:
Education: Bachelor's Degree in behavioral health related field
Experience: Two (2) years in behavioral health setting.
Training: Therapists providing Assessment, Treatment Planning and Psychotherapy to SA/MICA clients must have 60 hours of training specific to addiction services
Preferred Qualifications: Master's Degree
Supervisory experience preferred
Additional years experience in behavioral health setting.
Equipment Use: Computer, copier, facsimile
