Tracheostomy Coordinator is responsible for coordinating individualized tracheostomy care for patients throughout the continuum. Ensure timely access to healthcare services from the time of tracheostomy placement through decannulation. Independently evaluates patient’s level of function and determines, develops, and implements individualized treatment goals and plans in alignment with the patient/caregiver goals. Responsible for leading the Tracheostomy Progression Team and supporting timely and safe transitions of care that encompasses evaluation skills and techniques, analysis of patients, and airway management to maximize the patient's functional independence through the care continuum. Works alongside the Difficult Airway Response Team (DART) to support nursing, speech, and respiratory therapy with continuous education and competency. Participates in system wide multidisciplinary initiatives dedicated to the care of patients with tracheostomy. *Qualifications:* * Bachelor’s degree or higher in allied health, speech or respiratory therapy from an accredited program.Master's Degree preferred. * Experience with tracheostomy patientsUnderstanding of airway anatomy and needs of patients with surgical airways. * 5 years acute care work experience with acute medical, surgical, cardiac, neurological and critically ill patients preferred. * Active license in Respiratory Care or Speech Pathology in the State of Connecticut required. * Basic Life Support (BLS) (upon hire or within 30 days of hire) We take great care of careers.
