Facilitates communication between sending and receiving facilities, including the use of telephone, computer software, and other communication and documentation tools. Documents a record of the encounter.
Responsibilities
Answers telephones in accordance with established policies and procedures.Utilizes professional tone and demeanor in all communication. Coordinates communication between sending and receiving facilities, and within the receiving facility. Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner. Works in a collaborative manner with other members of the health care team. Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests, and numerous interruptions in a positive manner, establishing priorities for effective work completion. Able to diffuse challenging situations and respond appropriately. Demonstrates expertise in working with multiple computer applications as well as all functions of the telephone system. Escalates barriers and concerns using applicable policies and procedures as well as chain of command. Follows patient identification policies and procedures to ensure accurate patient identification. Utilizes downtime procedures for effective communication and documentation when necessary. Collaborates with Emergency Medical Services (EMS) to arrange appropriate and timely transportation based on established protocols. Obtains insurance information and communicates with insurance providers and/or case management and social services as needed. Initiates registration activities and completes registration as requested. Prepares handoff and other reports as directed. Maintains a clean and sanitary work environment. Other duties as assigned. Patient age specification: Neonate, infant, toddler, pre-school, school age, adolescent, adult, geriatrics. Must be able to sit and work at a computer with multiple monitors while speaking on a telephone for extended periods of time.
Qualifications
Education, Knowledge, Skills and Abilities Required:
High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.Education, Knowledge, Skills and Abilities Preferred:
