ESSENTIAL DUTIES AND RESPONSIBILITIES:
Serves as front-line liaison between outside referring facility customers and the UVA Health System for all non-intensive care and acute care level patientsUnderstands and properly uses all communication equipment, programs, and technologyMaintains a positive and professional work environmentIdentifies potential areas for improved operations and carePerforms telephonic patient triage and processes requests for assistance with acute patient transfers and/or admissions of patients from medical facilitiesDemonstrates the knowledge and skills to secure relevant patient care data and identify key patient issues; understanding the relationship between assessments, interventions, and patient responses; skilled in documentation, assessments and cognitive interventions.Provides medical interpretation of diagnoses and specialized patient care needs, recognizing critical and emergent situations, and connecting the referring physician with the appropriate UVA physician/serviceProvides clinical insights and overall level of care information to physiciansProvides health care access through the accurate gathering of demographic, sponsorship or guardian data, as well as insurance, financial and statistical information from a variety of sourcesDemonstrates the ability to triage and prioritize patient information and communicate an accurate clinical presentation of the patient for accepting physiciansIdentifies health needs based on assessment and utilization of guidelines. PHYSICAL DEMANDS
This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
