Job Description​

The Opportunity: 

  

Saint Luke’s Hospital in Kansas City, MO is seeking a Registered Nurse to join the Central Patient Logistics Transfer Team in a dual role as Patient Placement Coordinator and Transfer Team Nurse.

Shift Details: Mid-Shift 11am - 11pm or 3 pm until 3:30 am depending on the need.

Transfer Team RN: This RN position is self-directed and functions independently to facilitate internal and external physician access to SLHS. Demonstrates increasing discriminatory judgment in interpreting data and prioritizes appropriately. Utilizes the nursing process, requiring ability to work independently, with strong problem solving skills, critical thinking.

Work effectively and cooperatively with diverse members of the health care team.Communicates clinical issues and address these issues immediately via proper System channels.Serves as resource to colleagues at the unit or department levels.  Provides tele-nursing care to a diverse population of critically ill patients across the Saint Luke's Health System.Delegates task as appropriate and assumes preceptor responsibilities as needed.

We are seeking a detail-oriented nurse willing to serves as a tele-health nurse to patients discharged from SLHS facilities to evaluate, triage, and communicate with SLHS physicians, clinical staff, Care Integration and other clinical and social support as needed.

Job Requirements

Applicable Experience:

2 years

Advanced Cardiac Life Support - American Heart Association or Red Cross, Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various, Trauma Nurse Core Course - Emergency Nurse Association

Associate Degree - Nursing
