JOB DESCRIPTION
Position: Trans Health Navigator
Hourly Rate: $25.00 - $26.00
Hours: 40hrs per week, 8a-4p (Flexible schedule to accommodate required evening and weekend hours)
Department/Division: Adult Services FLSA Status: Non-Exempt
Location: Case Management Program/COPE Center. Supervised By: Program Director or designee
General Statement of Duties: Perform outreach, network with community groups and in-reach to recruit and engage transgender individuals. Provide risk reduction counseling, make recommendations and provide screening services as well as providing education and risk reduction strategies.
Responsibilities:
1. Perform health navigation services for transgender individuals in need, including supported referrals to address the full spectrum of individual needs and providing
appointment accompaniment. Ensure linkage to medical care, behavioral health treatment, PrEP, nPEP, disease intervention specialists, and other services.
2. Work with the Director of HIV Services to develop strategies for outreach, recruitment, and service delivery.
3. Screen, interview and counsel individuals who feel they are at risk of HIV/STI/HCV infection. Provide harm reduction information and strategies for at-risk individuals.
4. Collect specimens (via venous blood draw, finger stick, and/or urine specimen) for HIV/STI/HCV antibody tests from individuals who feel that they are at risk and send
and/or transport specimen as appropriate to the State Laboratory.
5. Provide education on gender affirming procedures, hormone treatment, and risk reduction strategies. Provide supported referrals and health care navigation for
transgender individuals, including for gender affirming health care
6. Collect data and complete paperwork timely and accurately for statistical and monthly reports.
7. Initiate and conduct follow-up activities to assess implication of services provided and the impact those services have on clients.
8. Develop, provide, and document appropriate referrals in focused areas for clients in need of services. Provide transportation to clients as needed.
9. Provide and distribute all appropriate materials, such as condoms, bleach, printed material that will increase the target population’s level of awareness and
understanding of HIV transmission.
10. Enroll appropriate clients and bystanders to participate in the Narcan Pilot Program.
11. Facilitate group level interventions and health communications with DPH/OHA approved curriculum as designated by supervisor.
12. Work in conjunction with the Program Director and persons served to assess the ongoing effectiveness of the Prevention, Counseling, Screening and Referral
Programs.
13. Coordinate efforts with other HIV/AIDS and transgender health programs.
14. Promote the program through community outreach and information dissemination. Network with community groups and agencies as needed to advance the
prevention/education models that are implemented.
15. Attend meetings, conferences and training to expand knowledge of HIV disease and related topics (STD’s, Hepatitis C, substance abuse, harm reduction, overdose
prevention etc.) and be current on new developments as required by supervisor or designee.
16. Follow a flexible schedule that accommodates required evening and weekend hours.
17. Complete all necessary paperwork and documentation in a timely and accurate fashion.
18. Establish and maintain good rapport and harmonious relationships with staff, clients, and other program related individuals.
19. Assist with case management responsibilities as needed.
20. Perform duties consistent with agency/program policies and procedures.
21. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
22. Perform other related work duties as needed or as required by supervisor or designee.
Qualifications:
• Transgender is a Bona Fide Occupational Qualification for this position and only transgender candidates will be interviewed.
• BA/BS preferred; or combination of experience and High School Diploma or GED plus outreach/education.
• Knowledge of HIV/AIDS and Harm Reduction principles and practices.
• Knowledge of Brockton area transgender community.
• Training in safe practices for blood draw preferred or will be trained.
• Good organizational and communication skills (oral and written) required.
• Demonstrated ability to take initiative and work in a team and individual setting.
• Approachable personality and open-mindedness required.
• Familiarity with the Brockton area required.
• Must have valid driver’s license and own means of transportation.
