Transition Facilitator (ATP)
 

Flexible - 20 hours 

(Saturday availability required)

The Transition Facilitator (TF) has a bachelor’s degree and supports Young Adults (YA) in transition planning and developing their life goals utilizing evidence based practices. The TF helps YAs recognize that life situations provide both successes and challenges that lead to valuable experiences and opportunities to learn. The TF will develop programming to help YAs with emotional/behavioral needs to continue their education, find a job, or live independently after graduation. The TF might also offer guidance or vocational counseling to help YAs achieve their academic or career goals. The TF will provide direct support in developing, adapting and modifying plans to meet the YA’s needs. The TF will be supervised directly by the Program Director and Clinician.

Qualifications:

1.Bachelor’s degree is required 
2.One year experience supporting young adults in transition planning required.
3. Practical experience working with young adults with special and diverse needs.
4. Basic understanding in mental health, SUD and trauma.
5. Have a basic understanding and/or willingness to be trained in TIP, AMP, and
Wraparound.
6. Excellent verbal and written communication skills.
7. Must possess a valid driver’s license and own means of transportation
