Job Details Transition of Care (TOC) The Transitions of Care Pharmacist will work collaboratively with healthcare professionals to maximize positive clinical outcomes such as minimizing risk of drug related adverse events, reducing hospital length of stay and reducing readmissions. The Transitions of Care Pharmacist will provide patient-centered care, education, and evidence-based, cost-effective pharmacotherapy. He/she will ensure smooth care transitions. Will participate in organizational performance-improvement efforts related to maximizing effective drug therapy while minimizing adverse drug events and hospital readmissions. Will serve as a resource to healthcare professionals and participate on institutional committees as appropriate Responsibilities: + Evaluate drug therapy regimens as noted previously. + Collaborate with unit staff, including nursing, managed care coordinators, medical staff and pharmacy staff to facilitate discharge planning for targeted patient populations. + Act as a liaison between care team members and the pharmacy department for medication related issues. + Work with patients and/or caregivers to identify and address barriers to medication adherence and cost. + Anticipate discharge date and discharge medications; anticipate potential issues that may occur including high-cost items, items not covered by insurance, compounded items, etc. + Collaborates with the interdisciplinary team to facilitate medication prior authorizations as needed for continuation of care. + Evaluate discharge medication orders and instructions, as appropriate, and provide clarification as needed. + Counsel targeted patients and/or caregivers on discharge medications. + Communicate with outside pharmacies to ensure a smooth transition for patients going home or to an alternate site. This includes working collaboratively with key stakeholders & across various disciplines to ensure patients are going home on safe, effective, and affordable drug therapy. + Contact patients and/or caregivers post discharge to answer any questions/concerns and identify any potential medication related issues. + Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, provide clinical consultation and clarification to practitioners and promote rational drug therapy selection. + Provide accurate drug information to professional staff, patients, and families. + Document all transitional care or medication therapy management interventions accurately and completely. + Detect and report suspected adverse drug events. + Develop and provide presentations, in-services, newsletters, and other documents related to transitional care for healthcare professionals, for patients, and for the community. + Read, extract and interpret information in patient charts accurately. + Monitor drug therapy regimens for contraindications, drug-drug interactions, allergies and appropriateness of drug and dose. + Discuss drug therapy regimens with prescribers as appropriate. + Actively participate in departmental, facility, and/or system-wide quality improvement activities + Actively participate in pharmacy research aimed at optimizing patient care outcomes, financial outcomes or operational outcomes for service line + Collect and maintain data on metrics that may be used to evaluate the impact of pharmacy services on quality measures and patient outcomes. + Serve as clinical clerkship preceptor and/or research mentor for students + Serve as Residency program coordinator for PGY-1 Pharmacy Residency program. + Oversee Residency Advisory Committee by setting up meetings and maintaining minutes of meetings. + Assist in overseeing preceptors in training with the Residency program director. + Serve as a preceptor for PGY-1 residents’ clerkship and or research mentor. Requirements + Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, and Doctor of Pharmacy. + Completed an ASHP accredited PGY-1 Pharmacy residency. + Obtain Board Certification in Pharmacotherapy (BCPS) or Ambulatory Care (BCACP) within 1 year of employment + Active NYS Pharmacy license, required + Previous experience in hospital pharmacy, required + Active Board Certification in Pharmacotherapy (BCPS) or Board Certification in Ambulatory Care (BCACP), preferred. + Transitions of care pharmacy experience, preferred. + Completion of a PGY-1 residency and two years of related experience within the past three years or PGY-2 in Ambulatory Care, Internal Medicine, or Geriatrics residency/training within past three years, preferred. Salary Range USD $63.00 - USD $82.80 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
