Nexus-Woodbourne is seeking a Transition Services Coordinator - Program 1 that has a high degree of attention to detail, and time management with discharge planning!
Position Summary:
Our Transition Services Coordinator works to facilitate the onboarding and aftercare services while coordinating the transition and discharge planning of Nexus clients while facilitating a positive and healthy environment that supports and emulates the Nexus philosophy and treatment modalities.
Pay rate starting at $24 - $26 per hour!
Primary responsibilities:
Client Admission, Transition & Discharge
Plans, organize and prioritize multiple assignments and priorities that makes best use of time; coordinate with school and residential teams to facilitate client transitions within the educational experience of youth on campus Facilitates effective training and presentations for staff including classroom presentations and development of corresponding support materials; demonstrates thoughtful and quality written and oral communication Collaborates with stakeholders to discern appropriate level of care, discharge placement and transition readiness for future placement and services needed, post-discharge Completes stabilization plan and provides transition/aftercare services to youth Reviews and interprets client information, aiding in educational and clinical teams to determine the best placement for youth.Resource Coordination
Provides individual one on one sessions to clients and their families when applicable; emulates a high degree of ethics and integrity and alignment with the Nexus mission and values at all times in all interactions Manages and coordinates the resources and activities for clients Collaborates with internal teams to identify the client’s need for treatment, skills, or education Maintains relationships with community partners such as schools, groups homes, transitional and independent living programs, community centers, and health clinics to create thoughtful referralsCompliance, Policy & Documentation
Completes detailed staffing reports to which document and reflect individual youth educational and vocational needs progress and barriers in addressing individual pertinent needs Collaborates with assigned agency professionals to complete transition and discharge paperwork Assists in transition visits/sessions and post-discharge follow-up services Ensures confidentiality regarding youth challenges, background, and treatment Maintains working knowledge, understanding and application of all Nexus policies and proceduresSafety, Collaboration & Positive Environment
Ensures compliance with all safety policies and procedures involving living areas, buildings, campus grounds and while attending off-campus activities Performs First Aid/CPR procedures and responds according to established protocols in the event of an emergency situation Maintains appropriate boundaries in all interactions and contacts with clients and their familiesRequired Education and Licensure:
Bachelors degree in related field required Minimum of 2 years of experience working with youth and/or families Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirementsPreferred Education and Experience:
