Transition Specialist

Wage/Salary: $62,400-$65,000

Position Summary:

Responsible for comprehensive culturally competent services to individuals detained in the city jails and upon reentry of IMPACT.

Essential Job Functions:

Conduct thorough assessments and through a motivational interviewing process, develop individualized person-centered transition plans.  Provide crisis intervention and engage in safety planning.  Provide evidence-based clinical supports as needed.  Facilitate care from Rikers through successful re-entry in the community. This includes ensuring that connections are made to appropriate internal and external services as required to meet the needs of the participants.  Conduct regular presentations of services in the jail. Complete transition plans for detainees in Rikers Expedite communication and/or case conferences with DOC, CHS, jail-based providers, defense teams, community partners and positive supports.  Maintain regular contact with participants to meet program standards and achieve program retention. Sustain open communication with program participants through face-to-face contacts, phone conversations, emails, and text messages.  Provide outcomes reporting.  Manage participant files, including progress notes and report on progress toward goals. Work in conjunction with Reentry Mentors and Reentry Coordinator to ensure seamless transition and continuity of caregiver.  Provide weekly reports to team and Assistant Director.  Participate in regularly scheduled supervision and on-going training. Attend meetings and participate in professional development training offered. Enters case notes for every job seeker in FedcapCARES.

Education and Experience:

Bachelor’s degree in human services, psychology or special education or related field from an accredited college or university; Master’s degree a preferred  Experience working with formerly incarcerated individuals preferred. Case Management experience preferred. Excellent facilitating skills  Extremely aware of his/her surroundings  Excellent communication, analytical and interpersonal skills. Must be computer literate and proficient in MS Office products. Highly organized and detail minded. Must be able to receive entrance clearance to Rikers Island. Bi-lingual (English/Spanish) preferred.
