Responsibilities Job Summary Responsible for the abstraction and entry of demographic, clinical, and trauma data through multiple automated systems, medical records, and concurrent rounding on critical care/telemetry patients according to HH Trauma Services and Arizona Department of Health Services standards. Performs case findings on trauma cases by reviewing and analyzing health records. Analyzes quality data to provide consultation to the medical staff leaders and multi-disciplinary health care team in order to continuously improve care, service and medical and nursing practice. Responsible for coordinating and developing the trauma outreach program through marketing, external program education, community hospital collaboration, and prevention activities. Collaborates with internal and external customers for educational opportunities while maintaining positive relationships. Acts as a liaison with referring hospitals and local organizations. Supports a collaborative role for the pre-hospital providers and referral hospitals. Design, implement and evaluate evidence-based injury prevention programs for HonorHealth Trauma Programs, the community and other professionals to ensure program effectiveness. Rounds on critical care/telemetry patients. Reviews charts for complications and quality issues. Abstracts and enters demographic, clinical, and trauma data through multiple automated systems and medical records. Provide data to Trauma Medical Director, Trauma Manager, Trauma Physicians, Nurse Practitioners and others to facilitate the recognition, research, isolation and resolution of identified opportunities for improvement. Performs case finding by reviewing for variances in care and opportunities for improvement and analyzing/assembling related patient information including charts, laboratory reports, and X-Ray reports. Serves as liaison to referring physicians and facilities. Coordinates and/or provides inter-facility or referring facility education as requested. Markets trauma center to these stakeholders and provides positive customer relations by maintaining regular communication via telephone, email, and site visits. Provides timely feedback letters/communication to the designated prehospital quality care coordinator and referring facility designee while ensuring HIPAA compliance. Maintains accurate records of all injury prevention and outreach activities. Remains knowledgeable on injury prevention legislation activity and evidence-based safety initiatives. Utilizing registry data, develops and coordinates injury prevention programs utilizing local, regional, and national programs. Represents the trauma center on local and state committees related to injury prevention. Train and orient interested staff to support injury prevention and outreach programs. Assist in development of policy and procedure for clinical operations in conjunction with trauma manager and trauma medical director. Participates in QA/QI activities to ensure accuracy of data collection and timely submission of data according to Arizona State Trauma Registry and the American College of Surgeons Optimal Guidelines for Trauma Centers. Maintains the quality of the registry program by participating in internal and external committees related to trauma services in Arizona and utilization of data. Serves as a resource and mentor to data support and analysts. Coordinates databases including conducting data entry, performing quality checks, monitoring database integrity and troubleshooting systems.
