Trauma Recovery Center Intake Coordinator - Trauma Services - St. Vincent Medical Center

Primary Function/General Purpose of Position

This role will provide intake and triage of eligible patients into the Trauma Recovery Center through patient screening, identification of patient needs, and provision of case management. They will provide counseling and psychoeducation to patients and is part of the Trauma Recovery Center Team approach to care.

Essential Job Functions

Provide initial screening and intake to identify eligible patients into the program.

Assess patient needs and works collaboratively with the Trauma Recovery Center Team to provide case management for victims of crime in our program.

Provide basic counseling to patients and screens for mental health symptoms.

Case management might include connecting patient to other social services, assisting with issues such as transportation, housing, immediate safety needs and pantry items.

Provide basic psychoeducation to patients on topics such as mental illness, coping skills and stress management.

Work with scheduling patients to maintain active participation in the program and to increase patient follow through of services.

Attend weekly team meetings with the TRC Team and participates in supervision on a regular basis and participates in trainings.

Provide individual and family psychotherapy (only with existing LSW licensure) which includes completion of psychosocial assessments. This would be a small portion of the job role.

Group facilitation

Successfully participates to complete BSMH and departmental specific training as required

Licensing/Certification

Licensed as a Social Worker in Ohio (preferred, required within 12 months of hire)
Experience:
• 1 year of experience in social services or human services setting. (required)

Education

Bachelors, Social Work (required)

Work Experience

1 year of field related work experience (preferred)

Training

EPIC Electronic Health Record (preferred)
