The WelbeHealth, theTraveling Physical Therapist is accountable for coordinating and providing restorative and rehabilitative physical therapy services to participants at multiple day centers and at participants’ homes as assigned. This position works in close coordination with the PACE Physician, rehabilitation staff, and other IDT members to maximize participant independence and safety, and to develop and restore function.

Essential Job Duties:

\n

· Conduct physical therapy assessments in our facilities and in participant homes to define care needs, preferences, and goals of participants

\n

· Actively participate in IDT meetings to develop participant care plans

\n

· Deliver and document physical therapy interventions in accordance with physician orders and/or as agreed upon in participants’ care plans

\n

· Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, as well as treatments such as massage, heat, sound, water, light, or electricity

\n

· Oversee the Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)

\n

· Provide training and instruction to staff, caregivers, participants, and families in the use and care of therapeutic equipment

\n

Job Requirements:

\n

· Minimum of five (5) years of progressive physical therapy experience

\n

· Minimum of two (2) years of physical therapy experience in a clinical setting with a frail or elderly population

\n

· Bachelor’s degree in physical therapy required; masters’ degree preferred

\n

· Ability to work independently with minimal supervision

\n

· Valid California Physical Therapy License, or proof of license eligibility

\n

Salary/Wage base range for this role is $129,969 - $155,963 / year + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
